Description:

This is a living will, professionally drafting by a large firm for their clients. It is two pages, but includes space for additional instructions beyond the boiler plate instructions included. It must be signed by the declarant, in the presence of a witness, who must also sign. In a couple of states, it must be signed by two witnesses. Copies should be kept on file with the declarant and his/her attorney if possible, and given to hospital staff if ever needed. Please note, it is in Word Perfect format.
